Unfortunately Physio-Forward are unable to offer home visits.
We regret that any appointment cancelled with less than 24 hours notice
will be liable to a charge of 50%.
Payment:
Direct payment by you:
Payment is required after each treatment.
We can accept cash, cheque and debit or credit card.
Private Medical Insurance:
Physio-Forward is registered with all major private medical insurance
companies.
We recommend that you contact your company prior to starting treatment to
clarify the following:
- If a GP/Consultant referral to physiotherapy is required.
- If you have an excess on your cover which is liable by you.
- If you have any restrictions on the number of physiotherapy appointments
you may have.
- If you have any other policy restrictions which may be imposed such as
number of sessions or timescale within which treatment should be completed.
We are happy to invoice the following companies directly, providing we have
all the necessary information at the start of your treatment:
- BUPA - Clinic provider number: 80009516
- AXA PPP
- Standard Life
- Norwich Union/AVIVA
- Cigna
- Pru Health
If your insurance provider is not listed then we would ask you for payment
at the time of treatment and will provide you with a receipt and the necessary
paperwork after each appointment in order for you to claim your money back from
the insurance company.
